I learned recently that INQOVI is the same way, NPO for min 2 hours before taking, and 2 hours after, that includes coffee.

How does fasting relate to CML treatment?

Some tyrosine kinase inhibitors (TKIs) used to treat chronic myeloid leukemia require fasting because food can affect how your body absorbs the medication. This means you may need to avoid eating for two hours before and one hour after taking your dose—a total of three hours without food each time.

TKIs that require fasting include:
- Nilotinib (Tasigna) - taken twice daily, requiring six total fasting hours per day
- Asciminib (Scemblix) - taken once or twice daily with the same three-hour fasting window

TKIs that don't require fasting:
- Imatinib (Gleevec) - take with food
- Dasatinib (Sprycel) - take with or without food
- Ponatinib (Iclusig) - take with or without food
- Bosutinib (Bosulif) - take with food
- Danziten (newer version of nilotinib) - doesn't require fasting

Fasting helps your medication work safely and effectively. Taking TKIs with food when you shouldn't can cause your body to absorb too much or too little of the drug, potentially reducing effectiveness or increasing side effects like heart rhythm changes. Tips for managing fasting requirements:

- Set consistent medication times (like first thing in the morning after overnight fasting)
- Plan meals around your TKI schedule
- Use reminders like alarms, calendars, or pill apps
- Prepare meals in advance to reduce stress

If fasting is making treatment difficult—causing exhaustion, interfering with your social life, or making it hard to stick to your medication—talk to your doctor. They may adjust your dose or switch you to a TKI that doesn't require fasting. Finding a treatment plan that fits your lifestyle is important since CML medications are often taken for many years.
