My wife's doctor said they probably wouldn't do a platelet transfusion until it got down closer to 10-20,000. My wife is in the mid 60,000s and bruises easier. They are more concerned with her hemoglobin (which is in the 8's). If it gets into the 7's they would probably start blood transfusions.

Dawn... I was dx w CMML SEPT 2022, platelets been my biggest concern so far, started at 67, went to 94, now as of Aug 2025 57 to 67. This can effect everyone differently, but for me, I bruise easily, bumps and dings are always a bruise, they clear up in 5 to 10 days, its alit like being on aspirin or blood thinners. If i get a cut, it clots, but little slower, so have to bandage quuck and watch. My OC has me on W&W, it seems levels at or below 25 to 30 is when treatment reqrd. You have to be cognizant and careful, can even get hematoma of skull if you bump head very hard. I would be happy w 70 to 100.

How is low platelet count treated in people with CMML?

Yes, low platelet counts (thrombocytopenia) are commonly treated in CMML. A platelet count of 70,000 is considered low and can increase your risk of bleeding and bruising.

Here are the main treatment options:

Platelet Transfusions
You may receive platelets from a donor to help improve clotting and reduce bleeding risks.

Growth Factor Therapy
Neumega (oprelvekin) is a medication that encourages your body to produce more platelets naturally.

Supportive Care
Your healthcare team will monitor your counts closely and may adjust other treatments to help manage this side effect.

Low platelet counts in CMML happen because excess abnormal monocytes crowd out the bone marrow's ability to make healthy platelets. Treatment focuses on managing the CMML itself while addressing the low platelet levels.

It's important to avoid medications like aspirin, naproxen, and ibuprofen, as these can interfere with your blood's ability to clot and make bleeding risks worse.

Talk to your doctor about which treatment approach is best for your specific situation.
