In chronic myelomonocytic leukemia (CMML), particularly in the myeloproliferative type (MP-CMML), high levels of blood cells can cause the spleen to enlarge. The cancer cells pile up in the spleen and stretch it to an unusual size, which is why removing fluid doesn't address the underlying problem.
The spleen continues to grow because:
* Leukemia cells keep multiplying and collecting in the spleen
* The disease process itself drives the enlargement
