Does a platelet count of 15,000 and an enlarged spleen indicate CMML-2?

A low platelet count and enlarged spleen alone don't confirm CMML-2. The diagnosis requires specific criteria based on blast cell percentages in your blood and bone marrow.

CMML-2 is diagnosed when you have:
- 5% to 19% blast cells in the blood, OR
- 10% to 19% blast cells in the bone marrow

You also need:
- High monocyte counts in the blood
- No Philadelphia chromosome (a gene change found in other leukemias)
- Abnormalities in different types of immature cells
- Fewer than 20% blast cells overall

A low platelet count (thrombocytopenia) can occur with CMML and may cause easy bleeding and bruising. An enlarged spleen is also common with various types of leukemia, including CMML, as cancer cells can accumulate there.

However, these symptoms alone aren't enough for diagnosis. Your doctor needs multiple blood tests over time and bone marrow samples to measure blast cells and rule out other conditions. Other issues like infections or vitamin deficiencies can also cause abnormal blood counts.

If you're experiencing these symptoms, it's important to discuss them with your healthcare provider for proper testing and diagnosis.
